Have you run into access requests not showing up upon a request made through the UI/APIs/Custom UIs?

I recently ran into an issue where I created a new JDBC source, set it up with queries, provisioning rules, schema, create policy and aggregations. The usual stuff.

Upon marking entitlements requestable post aggregation, I tried to request access to it via the request center and through the APIs (via a custom-built UI).

In both cases, I saw that the API response to access request submission returned 202 Accepted, and in the response I could see the generated Access Request ID.

However, when I go into the dashboard to find this newly generated access request, it was nowhere to be found. Super weird. I don’t know why this happened.

I tried recreating the source from scratch, no luck. Tried it on a different tenant of the same customer, but no luck there too.

Have you faced this? If yes, have you resolved it? How? Would love to get your thoughts on this.
