We are Facing below Provisioning Error while creating AD Account .

Ensure that all attributes specified in the Provisioning Policy are correctly mapped and added to the account. The issue may stem from the absence of the specified attribute in Active Directory .
Apart from this i feel some of the attribute format is not as per AD policy, as example samaccname, manager
I would look at the directory and see if the object is at the correct location and if the DN you have is perhaps incorrect.
