This was not showing in non-prod environment also. After seeing your message I refreshed and was able to see Forms widget.

Just refreshed and it is working again in Production and Sandbox

Does this apply to fedramp instances too as I do not see this widget at all? Maybe feature flags were not restored for fedramp instances ?

This is enabled in FedRamp instances, but Forms functionality is tied to either Business or Business Plus suites. Do you have Forms available to use, but the widget is not showing up?
If you have Forms, you could check that the widget is enabled in the Customize button in the top right of the Home page.
If you can let me know your tenant name, I can also dig into this on my end. Thanks!

It is checked on on the System Settings and I am able to create forms but do not see the widget at all.
