IdentityNow login page SAML error

Hi, I am facing one weird issue in our IdentityNow tenant. When I try to open my org IDN home page - https://{orgname}.identitynow.com/ui/d/mysailpoint it ask me to login via okta verify. most of the time this login process works fine, but sometime when post okta verify process, I am getting error message “An error occurred while processing your request. Please try again later. If this problem persists, contact your administrator.” and URL will reflect as - https://{orgname}.identitynow.com/r/default/error?errorCode=FailedSAMLAssertion

once this error page come, I simple open a new tab and hit the IDN homepage URL and it opens the home page without any further okta verify login process. Its happening in Sandox and prod env.

Does anyone facing similar issue, if yes could you please suggest permanent fix.

Do the Okta logs say why the SAML assertion failed?
