Hello,
We’re hoping someone can assist with an issue we are experiencing in SailPoint. Access requests are appearing empty, and although the requests move through the full approval workflow, the access is not assigned to the user after approvals are completed. Additionally, the requested items do not appear in the access request details. Screenshots are attached for reference. Any help would be greatly appreciated.

Hi @shashi31 - are these generated from Manage Access or are the programatically created by a rule or WF? Any recent upgrades or patches?

These were generated from Manage Access only.
Recent patching activity took place, but we have been observing this issue since before the patching.
