@shirbhatea there is no error, just completion status shows failed
check the logs, there should be something.
I have verified all servers nothing found
can you just paste the error logs from the server, if there are any, and also, can you please paste the screenshot of the failed access request . Can you also paste the identityrequest from debug please??
Also can you provide the access request xml from debug to check the issue

We are seeing these error in identityRequest XML and request has been completed after second level approvals completed
please help me provide the complete xml of IdentityRequest.
And as you said request is completed, did you receive manual workitem for the requested items ?
