I have created form and want to remove as mentioned screenshot. Please help if it is possible to remove and I need to put user at place of form.

hello @amanKsingh,
Go to debug page and open the form object.
As mentioned in the screenshot add the value for “pageTitle”.

That is the default behaviour of Sailpoint. But you may add some meaningful title to your form by adding pageTitle attribute,
and screenshot will look like:
