Getting error in the workflow action "form"

re using Action “Form” twice in one of our workflows. This workflow will be trigerred when any user request for a specific role in request center and the workflow will send a form link to the recipient (requested user) using the “$.trigger.requestedBy.id”. This workflow was running fine and it started throwing the following error from last week. We did not change anything in the workflow or FORM.
