Kindly suggest email template for source/application health status, that can be healthy or unhealth, where we can add to email of recipients
There isn’t a custom template for this need but you can enable notifications under Global ==> System Settings ==> System Notifications for changes in Source and VA status and you can set the recipient list:

Yes as @Irshaad_Laher_WS said, your problem can be solved. But a small change is that in “Notification Options” enable Sources too, which check s for the status of sources.
And if you need to customize the email, under “Global” → “Email Templates”
use the above two email templates.

That’s correct, it will send an email notification of a status change if the status has been in a new state for 15mins or more. it won’t send a notification on immediate change of status.

Yes, for the source health status notifications ISC uses Source Health and for VA’s status updates ISC uses Virtual Appliance Health email template.
