Creation of custom form

Hello ISC Experts,
I wanted to Create some custom form (ex: add identity & entitlements as attr ) on submit it should trigger approval process where

  1. if user selected entitlement abc approval has to go for ‘X’ approver
  2. if ent is xyz approval should be ‘Y’…like wise and fall back approval is some org admin
  3. upon approval submitted ent should add to the user
  4. on the rejection we should just send an email to user

so I need dynamic approval .

is there any way that I can achieve this customization
