Bulk IT Role import

Hi All,
Recently we have onboarded one web services application and that working as expected, for the application we have to create IT roles that quit more, count is 100+ i have search how to create those in bulk i got the CSV format. I have tried with that but getting below error,

[Line 4] Invalid CSV formating for line: [ADD Role, IT, yyy Document Type (xyz), Appname - yyy Document Type (xyz), BAO Approver for SHVN]

If anyone is having these CSV file please share here.

Operation Role Type RoleName Role DisplayName Owner
This the my CSV format.
