I was facing an issue regarding the approval, were not sent in the Nerm workflow for the create workflow of profile creation. The workflow contains the steps request form , approval form and the create. In the approval form I have marked the requester as the person to approve.

Since the requester is the approver, they may not see the item in the actions tab, I believe it will just proceed with the workflow. The approver has to be other than the requester. I will verify in my POC tenant. Use a different approver and then login as the approver and check in the actions tab.
On the other hand, if the requester is the approver why do you need an approval form. Just have a request form and then create action in the workflow.
