In an IdentityIQ 8.4p2 environment, the customer uses a custom Quicklink to create the work item for new role. While the approver views the work item, it displays like below and missing the details.

Not sure how you are building the work item, but the reason why Account Name and Application are blank is because your ApprovalItem is missing application and nativeIdentity attributes.
If you want to quickly test it out, open your WorkItem in debug and change this line to add application and nativeIdentity:

Hi, I also had a look, at the workitem.xml which you have shared, and can see the application and native identity is missing from the approvalitem section in your workitem.xml. The issue is how you are building your workitem. Can you share your workflow code, of secion where you are building the workitem, I can have a check, and suggest a possible fix.
With the guidance from Sunny and Naveen, we discovered that the work item being built is missing the application and nativeIdentity in the ApprovalItem, which is the root cause of the issue. Thank you both for the valuable pointers!
