My AD 2 way processing issue

Hi,
In my sailpoint we are having the NS as AD Account in application accounts every user will have the NS Active Directory application in application accounts in identity cube.
Daily we are fetching the Leaver Failure Report recent days we are receiving high count of Leaver Failures because NS Active Directory is still in active state even after Leaver Feature triggered.
It’s showing error with ‘Retry’
We are not able to add the sailpoint business roles to the user if the business role is having NS AD IT -roles, we are not able to see the NS AD Entitlements via Entitlement Catalogue when we search its showing “The system has encountered a serious error while processing your request.Report the following incident code to your system administrator : 12345”

can you share the logs or something else?
