We can see two transactions on the provisioning transaction table, one as “create” source LCM, second “Modify” type IdentityRefresh.

For the create transaction, status as committed as iiq us trying to create AD account and passing the mandatory AD attributes.

In Modify request it is trying to do the same thing but the error as mentioned in the post.

Do you have one or more DCs configured in your AD App definition? The error looks like you try to modify object before it’s created which can only happen in AD when for example - create operation will go to DC1 and modify operation will go to DC2 but faster than DC1 sync with DC2.
