I would like to know about recent changes, did you make any recent changes at Schema level.
Could you share more details.
@sureshbomm Please check your applications. In case your app object is corrupted, it will show the same schema id in multiple apps. Check that if you find so, please remove the id and save it.. or better get the app xml from your code repo and import it.

Could you please provisioning plan xml and schema details
It could be related with workflow cases, can you please check from when the issue is there and from there, please delete the workflow cases, from debug. Please try again post doing it.
