We faced an issue where I requested one role from request center but in account activity for that Identity, it is showing Status “Completed” with stage “Terminated” and working as “Workflow was terminated early”. Any suggestion ?

This common error, in your tenant check access request event trigger is subscribed and active or not, as in workflows as well. If there are any active triggers, then make it inactive or ask the owner to add specific filter in that.
