If due to intermittent failure at the target system account not created. When the connection establishes, on next refresh account should create automatically from role. Account gets created from single account refresh from postman not form 8AM/8PM refresh. Can someone explain reason for this?

The 8 AM refresh may not pick this up. But the 8 PM refresh should definitely reprocess the account creation if the user is part of the role and account was not created initially.
Which API are you using.? The CC refresh API’s are deprecated :

Have to manually by hitting this API.

It will be deprecated, but it seems they have extended it for some time.
