Problems with SDIM - trying to solve for Identities with multiple Accounts

We want to get to where tickets are being created in ServiceNow to remove entitlements from the designated Account belonging to any given Identity (this action would be resulting from a Certification Campaign) and tickets are also being created to disable a given Account when the Identity to which that Account is assigned is terminated (this action would be resulting from the Term Lifecycle State).

The issue we’re running into is that ServiceNow tickets are not being created (through the SDIM) when multiple requests are being made for the same Identity. This problem applied to two different situations: when a single Identity has one (or more) Accounts on 2 or more Sources and they are being Revoked at the same time, and it also applies when an Identity has multiple Accounts on the same Source.
