Hi @amanKsingh

For this case, you can use standard Access Request Status Report. Using this report you can filter by application, report, target identity, or status of the report.
You can found this report in Intelligence → Reports → Reports → Access Request Status Report

This report only filter the entitlement, not specific roles and roles of entitlement

Need to create a report which contain all the roles assigned to user who is requesting also who are approving this all things in report.

If i understood it correctly you need to write a report to get all the assigned roles for users along with the details of approvers?

if my understanding is correct then you can make use of OOTB Live Report “Access Request Status Report”.
