we are encountering an issue during task PIRM task execution specifically for the access requests with all filtered items as attached. Thus, the request has nothing to provision. For such requests, the completion status does not change to success and then causes long running PIRM task session ultimately causing DB block issue. For resolution, we are manually setting the status to success and then running the PIRM tasks.
I suggest you to check what happen when PRIM runs; check the logs and the administrator console.
Maybe you have a rule what cause the block or does something wrong or the workflow.
Can you confirm if the access is being assigned or not?
If not, then can you confirm are you using OOTB LCM or you are using customized version of it? If there is some customization then you need to isolate that why your access is being filtered out.
You may enable the log in workflow by setting trace=true variable.
