ServiceNow Service Catalog - Approval configuration

I am using ISC Workflow for ServiceNow Catalog approvals with a Manager and GG reviewer configured on the Access Profile. The request is correctly sent to the manager, but after approval the workflow shows

1.Manager X (Approved)

2. Manager X (Requested)

and remains in a Waiting for Approval state, making it appear as though the same manager must approve twice.
