LCM Modify request to add entitleement getting converted to Create request

When some of our users are making request their request are getting failed with Lazy initialization error and when wee check the logs we are seeing modify requests are getting converted to Create operation.
in our log4j we are seeing
2022-01-07 12:36:13,786 ERROR https-jsse-nio-11412-exec-2776 sailpoint.provisioning.ApplicationPolicyExpander:414 - Create request for existing link!

any pointers on how to find the root cause is appreciated…

Can you turn ‘trace’ on for the LCM Provisioning workflow? We should then be able to see the plan as it. comes into the workflower - and continue debugging from there.
